Here are five mistakes we see repeatedly, and how to avoid them.
1. Your Site Safety Plan doesn't reflect current site conditions.
An SSP filed six months ago may not match what's actually happening on the ground today. Every new phase, every layout change, every added structure needs to be reflected in your plan. If your SSP is out of date, you're already exposed.
2. You don't have a licensed SSM or SSC on site.
This is one of the most cited violations in NYC. Major buildings require a licensed Site Safety Manager/Coordinator. Buildings 7–14 stories require a Site Safety Coordinator. >15 stories require a Site Safety Manager. No exceptions. No substitutes. 3. Workers are on site without SST cards.
Local Law 196 requires Site Safety Training for all workers on major construction sites. 40 hours for most workers, 62 hours for supervisors. DOB inspectors check these cards. No card means the worker stops and potentially so does your job.
4. Your DOB filing is pending but work has already started.
Filing an amended SSP doesn't mean approval. Work tied to unapproved phases cannot begin until the DOB signs off. Starting early is one of the fastest ways to earn a violation and a costly one...
5. Nobody owns the SSP.
This is the big one. On many sites, the Site Safety Plan exists as a document nobody actively manages. When the original preparer is no longer on the project, the SSP becomes an orphan. No updates. No DOB communication. No accountability. That's exactly what a plan takeover is designed to fix.
